1.  A 44 year old fudge salesman man went on holiday recently to a sheep farm in Devon. Since then he has developed 1-2 cm reddish papules with a surrounding erythema on his hands.

 

 

2.  Five days previously a 23 year old female has returned to rainy old England from Thailand.  She has had an abrupt onset of fever, malaise, facial flushing and a retrobulbar pain which is made worse on eye movement, severe back ache and a lymphadenopathy.

 

 

3. A 30 year old man is out walking in a European forest when he is attacked from behind a bush by a fox which bites his head.  He is fine for the next two months but then starts to develop tingling and pain around the around the wound area as well as a fever.  After 9 days he has developed anxiety and is experiencing auditory hallucinations.  He also has a fear of air and water.

 

 

4. Whilst working in Northwick Park hospital you encounter a 40 year old man who has recently just returned from Southern Africa.  He presents with bloody diarrhoea, tenesmus and abdo pain, but despite these problems tells you that he has had a wonderful holiday and that the food was “to die for”. On examination of his fresh stool under a microscope you see motile trophozoites containing red blood cells.

 

 

5. Whilst on elective in Africa you encounter many people who are experiencing a low grade fever. On examination they tend to have splenomegaly, hepatomegaly and lymphadenopathy. Their skin is also rough and pigmented. A bone marrow aspirate smear shows L.donovani species.
